To have simulataneously 'held' titles they must be won and defended. In the other cases given one title had to be immediately reliquinshed believe it or not.

I couldn't think of the other guy, and would never have gotten it without cheating, but I was pretty sure that Sam won the Mexican heavyweight title in this fight.
